What payment forms do you accept?

Visa, MasterCard, American Express, and Self-Pay.

Do you take insurance?

 Blue Cross/Blue Shield, Ambetter, Health Advantage, and QualChoice

What is your cancellation and no-show policy?

All clients are required to have an active credit card on file to hold scheduled appointments.  A fee of $100 is charged for a cancellation with less than 24 hours notice and for no-shows.  Our incapacity to accurately charge insurance for the time you reserved but were unable to recover necessitates this policy.  Monday appointments must be canceled or rescheduled by the Friday before the appointment, at the latest, no later than the time the appointment is scheduled for (for example, a 10:00 am Monday appointment must be canceled by 10:00 am the Friday before).
